using the toll free number on the back of your membership card to call
customer service or by using our online provider listings. Our provider
list is updated daily.

Hidden fees, such as administrative fees for each use of the card, may
reduce or effectively erase the advertised discount.
Colorado Unified Health does not have any hidden fees. The only fee that
is paid is your monthly or yearly program agreement.
- Will I be required to prepay services or pay at the time of
service?
Colorado Unified Health Program requires that you pay at the time of service.
- Will I receive discounts for emergency services?
Colorado Unified Health Program requires that members register within
48 hours of emergency room admittance. A representative will coordinate
all of the resources and negotiate with the provider/facility for a contracted
savings.
- Will I receive discounts when traveling?
Colorado Unified Health Program has a nationwide provider network.
- Do health care services have to be preauthorized?
Colorado Unified Health Program requires that hospital visits members
MUST pre-register before any procedure(s) are performed. A representative
will coordinate all of the resources and negotiate with the provider/facility
for a contracted savings.
- Will I receive regular statements outlining my savings?
Colorado Unified Health Program members must ask the practitioners themselves
for the savings.
- Can I pay the fee monthly rather than on annual basis?
Colorado Unified Health allows you to choose monthly or yearly.
- Is the membership fee refundable if I cancel the membership
at any time?
The member can cancel at anytime they have an active membership. Colorado

- What do I have to do to cancel my membership?
Colorado Unified Health Program requires that you call the toll free number
for customer service to cancel your membership.
- Does the program have a toll free number for a 24-hour customer
service line or web site? If not, during what hours can I call for assistance?

Colorado Unified Health Program may not have a specific practitioner that
you want although we do have over 350,000 in the program. If you have
a specific doctor that you require call our customer service to identify
if they are a part of our program.
- What type of discounts can members expect to receive?

- How does the doctor know that they are to give me a discount?
Colorado Unified Health is part of a national provider network and they
have agreed to accept the card. The information you provide them from
the back of the card identifies the network.
- Will all of the providers recognize the Colorado Unified Health
name?
Colorado Unified Health provider network is over 350,000 practitioners
so they may not recognize the name but the information on the back of
the card will identify the program.
- Will the Colorado Unified Health Program work even if I have
insurance?
Colorado Unified Health Program may be used in conjunction with services
not covered by your current insurance, with policies that have high deductibles
or that charge flat-fees for hospital stays. There are many types of insurance
coverage and it is not possible to offer advice on how the program works
in combination with all of the various types of insurance coverage.
