    Colorado Unified Health is part of a national provider network and they have agreed to accept the card. The information you provide them from the back of the card identifies the network.

    Colorado Unified Health provider network is over 350,000 practitioners so they may not recognize the name but the information on the back of the card will identify the program.

    Colorado Unified Health Program may be used in conjunction with services not covered by your current insurance, with policies that have high deductibles or that charge flat-fees for hospital stays.
